I can't show you the code on the Voice of the Internet site as promised because there is too much database stuff wrapped up in it. But I can show you the upload.tpl file I used in my class, modified slightly for use on a little multimedia archive site I built. Nice thing about this page is that it was very well commented for my students as it was part of an assignment. To access the raw code, go to http://gonzo.ofthedayclub.com/ and click on the upload link.
