I decided not to dink with it so I have the as two pages - the first would be the "other" 15 fields for you and then the second would be where it asks for the file. I append the first 15 fields to the database on the second page and then do a replace to add the file name after the second page is submitted.I used a modified version of Alain Russell's "fileupload" code to accomplish the actual upload.
